Change of Address
Please be sure to send us updated address information (click here for more information), with a notice signed by the artist, so your payments reach you in a timely manner. A signed W9 is also a satisfactory form of notification. A W9 form can be obtained in pdf format from the IRS's web site: http://www.irs.gov/pub/irs-fill/fw9.pdf. Please include a phone number so that we can contact you if additional information is required. If you are a foreign vendor, a completed W8Ben form is required which can be obtained at http://www.irs.gov/pub/irs-fill/fw8ben.pdf.
